When you purchase this service, we become your central contact to nurses, therapists, social workers, pharmacists and other specialists accessible nationwide. Your Care Advocate will contact your selected service providers and arrange for delivery of services to you.

During this phase, we accomplish two things:

• Implement the recommendations, based on your choices of products and services

• Follow up with you by phone two weeks after initial plan implementation to assess the effectiveness and appropriateness of the services

Plan implementation is provided for an hourly fee.
